Client Waiver and Consent:

The services and programs offered through Jill Flathmann are not meant to diagnose conditions, nor prescribe or perform medical/veterinary treatment, prescribe substances, nor interfere with the treatment of a licensed medical/veterinary professional.

The session(s)/program(s) are not a substitute for medical, veterinary, psychological, and/or other licensed professional treatment. The service(s)/program(s) offered from Jill Flathmann may compliment any medical, veterinary or psychological care you and/or the animal(s) involved may be receiving. It is strongly recommended that you and the animal(s) involved see a licensed physician, veterinarian, and/or licensed health care professional for any physical, psychological and/or spiritual ailment you and/or the animal(s) involved may have.

The session(s)/program(s) is information to be studied, diagnosed and treated by a licensed healthcare provider, as the session(s)/program(s) is not a substitute for veterinary/medical/psychological, licensed healthcare provider advice.  Jill Flathmann will not be held responsible for any unexpected behavior, injury, damage or loss resulting from the session(s)/program(s), nor does the session(s)/program(s) claim to find lost animals, change animal behavior, and/or heal sick animals and people.

Long term imbalances in the body and mind sometimes require multiple sessions in order to facilitate the level of healing and relaxation needed by the body, mind and/or spiritual aspects of you and the animal(s) involved.

You agree to hold harmless all shareholders, directors, heirs, successors, administrators, employees, and/or insurers from and against any liabilities, claims and/or demands which may be made against them by you, any personal representative, spouse, or beneficiary arising from the undersigned’s and animal(s) involved in attendance and/or session(s) with the service(s)/program(s).

The information from the communication may augment your understanding of yourself and the animal(s) physical, mental, emotional, and spiritual being, but medical diagnosis, treatment, therapy, and advice must come from a licensed professional.
